Helping Patients and Families with the Hospice Benefit

Sun Valley Hospice provides comprehensive, compassionate care for persons with a life-limiting condition or illness. Our interdisciplinary team uses a holistic approach to promote the patient's comfort and dignity. Recognizing the family as the unit of care, we support patient, family and caregivers by addressing physical, psychosocial, and spiritual needs. Care is provided wherever the patient lives; including the patient's own home, residential care homes, skilled nursing facilities, assisted living or independent living facility.

Eligibility for Hospice Care

To qualify for admission to the Sun Valley Hospice program the patient must:

-Be certified by an attending physician and the hospice medical director.
-Have a life-limiting condition with a prognosis of six months or less if the disease runs its natural course. These conditions include: Cancer, end stages of lung heart and neuromuscular disease, the final stages of Parkinson's disease, Alzheimer's disease or other debilitating conditions or multiple disease processes associated with aging. 
-Agree that the focus of medical treatment is for comfort or palliative care.
-Have a primary caregiver, family member, relative, friend or hired staff available if needed.

The Sun Valley Hospice team will conduct a patient evaluation at the request of the physician and assist the family in decision-making and caregiving/resources arrangements.

Some patients who are not ready for hospice care and want to continue curative therapies may qualify for Home Health services. Sun Valley Home Care is available for these patients.

Hospice Services Available

Hospice care is provided on an intermittent basis 24 hours per day, seven days a week based upon approved Plan of Care involving the patient/family/caregivers, the attending physician and the hospice team. Prescription medications, supplies and medical equipment related to the patient's terminal illness are provided at no cost to the patient or family.

-Pastoral Counselors
-Speech, Occupational and Physical Therapists
-Dietitians
-Bereavement
-Volunteers

Four Levels of Care:

-Routine Home Care
-Continuous Care 
-General Inpatient Care at a contacted facility
-Respite Care at a contracted facility 

Cost of Services

Hospice Care is covered by Medicare and AHCCS. Most major insurance companies also have a hospice benefit. Please contact Sun Valley Hospice for more information or to request a hospice evaluation.
